Support audio.transcriptions.create(stream=True) for sync and async OpenAI clients, including deterministic span finalization on stream exhaustion, context exit, and early close. Add VCR-backed regression coverage across the supported OpenAI SDK matrix for transcription streaming and affected partial stream context-manager behavior.
