Schema versioning & structured errors

• All --json outputs include schemaVersion: 1 and codesessionVersion for forward compatibility
• Structured error shape: { error: { code, message } } — parse error.code, never string-compare

Git-based file/commit accuracy

• startGitHead captured at cs start
• On cs end, runs git diff --name-status and git log from start HEAD to backfill accurate file/commit counts — even in agent mode where watcher isn't running
