Add namespace annotations opt in/opt out

Add ability to operate as opt in or opt out. When operating in opt in, only resources in namespaces with patchwork/enabled: 'true' will be considered. When operating in opt out, all namespaces without patchwork/enabled: 'false' will be considered.

At the resource level any objects annotated with patchwork/enabled: 'false' should be ignored.
