This PR adds the upgrade height for Mocha testnet v4 network upgrade as specified in the issue.

how-to-guides/network-upgrade-process.mdThe change ensures that node operators and validators have the correct upgrade height information for the pending app version 4 upgrade on Mocha testnet.
