Fixes a real defect: a visitor who declined was re-asked on the same schedule as one who accepted, and was tracked again in the meantime.

A refusal is now stored for 400 days and refreshed on every visit, so it never lapses for a returning visitor. Only a full acceptance expires and is re-asked. Accept and Decline lifetimes are separate settings.
