Universal Doctor Intelligence and Proof-Backed Polyglot Graphs

This release gives Doctor one causal, fail-closed diagnosis vocabulary and makes
persisted Graph retrieval live-source-aware across linked, native, and polyglot
projects.

What's New:

• One Doctor truth model

  • Findings carry stable identity, confidence, proof, causality, repair
    disposition, unknowns, contradictions, and completeness.
  • Applicability and freshness remain explicit, so absence is never counted as
    health.

• Portable capability evidence

  • 17 runtime adapters and 44 framework identities publish their support,
    limitations, platforms, and repair modes.
  • Summary and receipt artifacts give IDEs, CI, and agents bounded handoffs
    without discarding the complete evidence.

• Exact repair routing

  • Remediation retains causal finding identities across projects.
  • Missing init, test, build, or start evidence points to its exact producer
    command rather than a generic Analyze action.

• Safe, fast Graph retrieval

  • Read queries reuse the persisted graph only when its model binding, proofs,
    project scopes, and Git/Merkle live-input fingerprint still match.
  • Physical-path canonicalization preserves the Git fast path across macOS
    /var aliases and Windows workspace junctions.
  • --refresh-graph forces a live rebuild, while --scope project:<name> keeps
    retrieval and benchmarks project-focused without hiding connected shared
    APIs, schemas, owners, pipelines, or protocol entities.

• Native and cross-language architecture evidence

  • C/C++, CMake, Meson, Bazel, Protocol Buffers, language bridges, runtime
    units, and lifecycle stages now produce proof-backed graph facts.
  • Re-adoption refreshes stale runtime/framework metadata, and linked external
    projects resolve through their portable external/<project>/... identity.

• Bounded consumer payloads

  • Agent search publishes deterministic omission budgets instead of returning
    an accidentally unbounded one-hop neighborhood.
  • Graph benchmarks can read linked-project proof artifacts and report
    unreadable inputs explicitly.

• Stable extension compatibility

  • Backward-compatible CLI releases no longer raise the extension's minimum
    supported CLI version automatically.

• Faster local quality gates

  • Pre-commit now checks staged sources plus relevant documentation or contract
    drift without generating or staging files.
  • Pre-push keeps the complete local quality boundary while reusing one build,
    one full test run, and one pass over each conformance family.

Breaking changes: None. New evidence fields and contracts are additive.

Verification:

• The restricted-environment suite produced 2,413 passes and 8 explicit skips;
  seven loopback-listener cases were environment-blocked, and their three
  owning suites passed 60/60 with loopback access.
• 234 focused graph, adoption, lifecycle, consumer, and contract tests passed.
• The 170-case Doctor validation corpus passed across all 17 adapters.
• A linked gRPC repository produced 4,692 entities, 5,287 relations, and 7,332
  proofs with 100% proof coverage and no conflicts or unknowns. A bounded query
  reduced the estimated payload from 2,644,948 to 4,665 tokens (99.82%; results
  vary by workspace and query).
• Type checking, linting, formatting, contracts, parity, and docs passed.
