Reworks the trailing column so the working directory is always visible, moves the worktree
name onto the branch, and fixes the effort word drifting off the pipe when extra-usage
credits are high.

Fixed

• Effort word right-aligns to column 1's edge (both): the reasoning effort now sits flush
  against the first |, stacking directly over the extra-usage dollars below it. Previously
  a wide $spent/$limit figure widened column 1 and the generic padding was appended after
  the effort word, pushing it away from the pipe. The padding now falls between the model name
  and the effort — the same right-align treatment the dollars, token %, and Fable % already use.

Changed

• Worktree name moved onto the branch (both): in --worktree sessions the worktree name
  now rides on the end of row 1's branch as @branch:worktree (the colon and name hide together
  when there is no worktree), instead of occupying its own cell in row 2.
• Row 2's trailing cell is now the full working-directory path (both): it shows the complete
  path with your home directory collapsed to ~ (e.g. ~/projects/hw-radar), replacing the
  standalone worktree cell. It shares the trailing column with row 1's folder name, so both cells
  appear together or omit together when there is no working directory — row 2 no longer carries a
  - placeholder there.
• Fable cell shows 😢 instead of - when unavailable (both): Fable leaves subscription plans
  on 2026-07-07 and is expected to return later. While its weekly limit is absent, the Fable
  label stays put and the percentage becomes a 😢 rather than collapsing to a dim -, so the cell
  holds its column until Fable comes back. The emoji is treated as two terminal columns wide, so it
  right-aligns to the column edge exactly like a percentage and the pipes stay aligned.
