Edit mode tool approvals get stuck in "pending approval" #61
Description
Scenario: I previously selected auto-approve earlier in the chat. It looks like whenever an auto-approved tool call is made, it very quickly flashes in a pending approval state before switching to approved. Sometimes it gets stuck in the pending approval state and doesn't advance without a follow-up prompt.
