Refresh review window after agent completes fixes

Currently the review window stays stuck on the old view after the agent completes fixing the review.

We should indicate to user that this view is outdated and add a Refresh button.

There is a Refresh button (and Ctrl+R) key-bind. But perhaps it should be more prominent?

Auto refresh is challenging due to the way we implement the Review as a bunch of commands over the IPC. It has the potential to be a major resource hog.

My hope is to add inotify support at some point which covers both agent tool call edits and other actions (like formatter) that modify working tree.
