Codex request failures now show their error message and code instead of looking like an agent rested without signing off.

• Preserve terminal provider errors in the transcript and board, including policy rejections, authentication failures, limits, context errors, and unknown future codes. Show live reconnect notices separately from terminal failures. Failed turns no longer trigger sign-off reminders or rest Goal nudges. 187ffe47

Verified with a real Codex authentication failure and a browser test against the published npm package, including restart, recovery, and both fonts at desktop and narrow widths. This release changes Frizz's reporting, not the provider's decision to reject a request.
