fix: restore missing theme-switcher index.html (issue #73)

I noticed that after PR #73 was merged, the index.html file for the theme switcher was accidentally removed.
I’ve created a follow-up PR (fix/issue-73-restore-html) to restore the missing file.

This PR only restores the HTML file and does not include any other changes.
