- buildDomainContext: AGENT.md always in full (<=40k) + own topic budget
  (16k/8k) — fixes the 6000-char cap that silently dropped all topics.
- selectTopics: weight filename/headings over body + plural match + top-5,
  so a small focused topic beats a giant catch-all.
- runPlanned: the planner now receives the domain pack (was planning blind,
  emitting "inspect the workspace" instead of using the documented tool).
- bump agent-working-memory -> ^0.9.1 (import last_activated fix).
- groundwork for agent knowledge-store contributions (experimental, unwired).
