Split the shared index wire-format model and IRI path mapping helpers
into core/src/index/{model,iri}.
The code is shared by the index environment reader and the new index
management commands, so a domain module is clearer than index_utils.
Keep the existing sysand_core::index command API available and re-export
ParseIriError from that module.
