Refactor Plex ingestion to stream paginated batches

Summary

• paginate Plex movie and episode ingestion so batches stream without materializing full libraries
• keep existing batch sizing and logging semantics while falling back to computed totals when section counts are unavailable
• expand ingestion stage tests to cover paginated mocks and large library scenarios, and bump the project version to 1.0.13
