Inconsistent indexing and formatting of typedef declaration

In the index we have, under "declaration":

[dcl.pre] defines "typedef declaration" in plain text, and [dcl.typedef] uses it too; [basic.def] uses "typedef declaration". The formatting should be made consistent and the index entries merged.
