Investigate "device lost" Heisenbug

Hard to repro. Keep loading heavy meshes, sometimes the app will hang and crash due to the Vulkan device being lost.

Possible causes:
- Multi threading errors
  - Validation should be catching concurrent use of resources?
  - Fence status / wait has caused issues before: do a pass on these
- Driver bug
  - Try to repro on Windows
