Add event records for license renewals, deactivations

Insurers and others will want to know, for a given day in the past, whether an individual was licensed to practice in a given jurisdiction. To do that we will need to present historical information on an individuals license status over time in the detailed view. We need to capture important license events such as the dates of renewals and any deactivation initiated by a board, combined with license the expiry date associated with each event, so we can have a complete day-by-day accounting of their licensure.
