Added:
- autoExclude parameter (default: true) filters 16 lock file patterns
  from diff output to reduce context bloat
- excludedFiles output field reports which files were filtered

Fixed:
- Untracked file insertions/deletions now included in diff stat totals
- LOGS_DIR no longer defaults to 'logs/' — file logging is opt-in only,
  preventing crashes in npx/plugin environments (#38)
