Rewrote the git_wrapup prompt as a thin session-flow wrapper around
git_wrapup_instructions instead of the pre-v2.12.0 phased procedural
script. Dropped skipDocumentation and updateAgentFiles inputs (now
standing guidance in the protocol), flipped createTag default to 'true',
and stopped passing the removed updateAgentMetaFiles key to the tool.

Also verified @hono/node-server 2.0 is clean against our usage — both v2
breaking changes (dropped Node 18, removed Vercel adapter) don't apply.
