Add public hosted endpoint

Public Streamable HTTP instance at https://openchargemap.caseyjhand.com/mcp — no install required.

Added:

  • remotes[] entry in server.json (type: streamable-http)
  • ### Public Hosted Instance subsection in README Getting Started
  • Centered hosted-server link in README header

Changed:

  • vitest ^4.1.8 → ^4.1.9 (patch)

38 tests pass; bun run devcheck clean.
