DOI→PMCID resolution, config-aware tool description

The dois input branch now calls the PMC ID Converter before PMC EFetch, mirroring the pmids path. A DOI whose article is in PMC returns structured JATS instead of a false unavailable. (#64)

Tool description is composed at startup from the enabled fallback tiers (EUROPEPMC_ENABLED, UNPAYWALL_EMAIL), so tools/list only advertises what the deployment can deliver. (#65)

785 tests pass; bun run devcheck clean.
