Better observability for forked workflows, parent/child relationship

- Add started at completed at in operation_outputs (which we use to track parent - child relationships)
- Add forked_from
- Add missing fields to Conductor response (queue properties, fork information, step started at / completed at)

• Add connect_args with the application name for the client to pgx connections
