和 #4501 同一条：Chat 视图 conversation Context … received more than one start Match。通常是模型复用 read:0 / edit:0 当 callId，不是磁盘 seq gap。

dsh-session-surgeon 修不了这个 UI 折叠。先 inspect <id>：health=ok 就不要 apply。真正 seq-gap / torn / 缺 message.id 才是它的范围：
