[Bug] isQuotaExceededError wording table misses two real quota shapes: AllocationQuota.FreeTierOnly and in arrears

[Bug] 额度类失败的措辞匹配漏掉两类真实回包：isQuotaExceededError() 对它们返回 false（与已采纳的 #7052 同一个函数、同一量级）

Summary：packages/llm/llm/src/error.ts 的 isQuotaExceededError() 用五条措辞正则判定终态额度，但境内两家 OpenAI 兼容网关的真实回包里，有两类高频措辞一条都不落。后果不是重试风暴，而是这类失败在判序修正之前连"是不是额度"都判不出来：403 那支先短路成 AUTH（见 #3222），而 402/429 那两支靠这个函数兜底，函数返回 false 就退化成 HTTP_<status> 或 RATE_LIMIT。

复算方法（不需要账号，直接调发布产物里的函数）：

0.1.7-rc.1 上的读数（同一函数、同一份五条正则表，lib/index.js:179）：前两条 false，后两条 true。也就是说 quota exhausted 与 insufficient balance 这两类已经被认出来了，缺的是「免费额度用完即停」与「欠费/拖欠」这两种境外网关不会用、境内网关默认用的说法。

Proposed fix（与 #7052 同量级：一个函数里加两个分支，不碰状态码语义、不碰 DEFAULT_RETRYABLE_CODES）：把 FreeTierOnly / free[\s_-]*tier[\s_-]*only 与 in arrears|arrearage|overdue payment|no available (credits|quota) 收进这张表。它们都是"换路由或充值才能恢复"的终态措辞，不该落到 RATE_LIMIT 或 HTTP_<status> 上。

Notes：

• 这条不主张改判序（403 排在额度之前是 [Bug] HTTP 403 授权拒绝被归类为 AUTH，UI 显示 "API key is invalid"，原始错误不可见 #3222/[bug] 403 quota exhaustion shown as "API key is invalid": every AUTH-coded failure swallows the real message #5715/Bug: pi-ai adapter misclassifies 401/403 in error text as AUTH, surfacing misleading "API key is invalid" #3073/Fix: classify 401/403 bodies by semantics (context overflow / quota) instead of always AUTH #1127/【BUG反馈】额度耗尽（403）被统一显示成 "API key is invalid" #1631 的内容，那边已经在讨论，我不重复）。判序改了也仍然需要这张表补齐，否则 402/429 那两支照旧漏。
• 我方能证的是函数级 false 与两条真回包文本；不同网关的字段位置不同（有的放在 code，有的放在 message），所以正则表比状态码白名单更该补 —— 而 detail 恰好是把 type + code + message 拼在一起送进来的，两条都能命中。
• 判据（盯代码，不用等回音）：上面那段 node 一行，前两条从 false 变 true 就结案。
