Plugin update v0.1.2

Same reference plugin now uses soft/hard tokenize pressure:

• soft (thresholdRatio): log + allow (avoids fail-loops when this-turn files dominate)
• hard (hardRatio, default 0.90): synthesize CONTEXT_WINDOW_EXCEEDED for stock compact-retry
• overflow rewrite still global

• Hard pressure: keep an output reserve (default 4096) before synthesizing overflow
