PLACEMARKER in Discord notifications

Can someone hopefully decipher what is causing all the PLACEHOLDER spam in these Discord notifications?
Is this something I'm doing wrong with the AppRise notification?

The notification URL I am using is:

discord://Change%20Detection@webhook_ID/webhook_token

what notification format is it set to?

Thanks for pointing me in the right direction - I was trying to pass HTML instead of plain text.
