Auto-resolved by the prior commits: cwdForHeader in terminal-state.ts collapsed to "running uses cwdAtStart, everything else uses pane.cwd" (da25a4c), and layout.md now delegates the disambiguator rule to terminal-state.md (2aaa16c).
