- add check for fFindUnqualifiedAccesses and
  fFindUnqualifiedMethodAccesses in CodeStyleFix.visit(TypeDeclaration)
- this patch also fixes the same problem when referencing instance
  methods of inner classes of interfaces
- add new test to CleanUpTest
