* Fix lambda cleanup/quick assist when static field is returned

- fixes #1209
- fix LambdaExpressionsFixCore to create a QualifiedName reference
  rather than a QualifiedType
- add new tests to CleanUpTest1d8 and AssistQuickFixTest1d8
