Backport fix from 4.3:
  * Fix and rename MenuHelper.getIconUrl() to lookup failed icons
    in the workbench images
  * Adapt other locations doing ad-hoc icon URI resolution to use
    MenuHelper.getIconURI().
  * Add tests for MenuHelper
