Current Rails version uses `README.md` instead of the old `.rdoc` file,
and the templates have not been updated to reflect that.

This patch changes both the logic to insert content into the README file
and the check whether a previous template has been already run.

Thanks!, merged! I've missed this change in latest Rails, so I've fixed the other templates as well, both the logic to insert the content and the logic to check whether a previous template has been already run.
