Add "Uploading the packed image" section to docs/qemu.md covering the
full flow: pack → gh release create on the private images repo → bump
DISK_TAG. Includes the constraints the download script imposes (single
zip asset, flat archive layout).

Fix stale DISK_URL reference in pack-disk.sh header comment.
