Software "Edit appearance" form shows two "Choose file" buttons

Steps to reproduce:

1. Navigate to Software page
2. Click on a software link
3. Open "Actions" dropdown
4. Select "Edit appearance" option
5. Observe choose file UI
   Expected: Only one "Choose file" button should be visible.
   Actual: There are two "Choose file" buttons.
