docs: tighten bridge ops and roadmap priorities

• tighten Claude Code local bridge guidance in README and bridge docs
• replace the sprawling roadmap with a cleaner current-first product roadmap
• add a concrete implementation plan for the next release lines
• define Anthropic, Claude Code, and Claude Desktop parity as separate targets
