docs: add /design-consultation skill to README#127

The /design-consultation skill was fully implemented but completely missing from the README.
This PR adds comprehensive documentation: it's included in the skill table with description, the install/uninstall instructions list it, and there's a detailed section explaining how it works (design system from scratch, competitor research, preview page, DESIGN.md generation) with a usage example showing the conversation flow.
The skill builds a complete visual identity when you start from zero, contrasting with /plan-design-review which audits existing sites.
