when reform inserts features into a host chromosome, it shifts coordinates but doesn't relabel the seqid

Do you mean the ID of the chromosome?

yes the chr ID of the inserted sequence was not updated. my inserted seq annotation file was ID-ed synchr , and it did not update to chrX after I inserted it there.
