File Read error from session state using codex 5.2

Describe the bug

While using copilot version 0.400 and CODEX 5.2 on HIGH thinking, more often than not is see this error -- it eventually figures it out

Affected version

GitHub Copilot CLI 0.0.400.

Steps to reproduce the behavior

1. Provide Codex with a task that spans across multiple files and involves executing a written plan.
2. Make sure the task is beefy.
3. You should observe the same error attached.
