[storify] Gateway repair and overnight workflow recovery
#68992
A gateway startup defect turned one publication boundary into a fleet-wide interruption overnight. In the local snapshot, 92 detailed run summaries across PR review, scheduled, release, and smoke workflows failed at Start MCP Gateway between 22:24 and 00:40 UTC. These were agent-job failures; the agent often never began. Issue #68881 identified an undefined optional repository value, and PR #68882 was merged at 00:46 UTC after review and revision.
The visible trajectory then changed. Later PR review and Daily Go Test Parallelizer runs reached successful agent and conclusion jobs. This is strong evidence that the shared startup blockage cleared in representative workflows, though it does not establish every workflow is healthy. Coverage is partial: 371 local run records have 283 detailed summaries, and the additional 24-hour logs request stopped at the storage limit.
Release needed another turn after the gateway fix. The first dispatch failed at startup. Two later dispatches failed in Create or update tag, a separate step whose cause is not established here. The 04:19 UTC dispatch, §38680824229, completed its tag, release, agent, and safe-output jobs and recorded an update-release output. Outcome: recovered for this dispatch; medium confidence about the broader cause of the tag failures.
Issue Arborist resumed linking. Yesterday's PR #68623 repaired targeting for scheduled sub-issue links after a failed run. Today's §38686267247 completed with five link_sub_issue outputs and one discussion. Outcome: improved in one observed scheduled run; continued success remains to be seen.
Feedback Loops Across Workflows
The Publication-boundary repair loop is improving: failed outputs or startup exposed defects, a person narrowed the contract, and merged fixes preceded successful outputs. The gateway sequence connects multiple workflows through #68881 and #68882; Issue Arborist shows the same pattern at the sub-issue target boundary.
The Failure-to-investigation closure loop remains mixed. Automated failure reporting reached the 50-issue daily cap and continued recording suppressed failures in #68910. One Failure Investigator run, §38677344534, stopped at its activation token guardrail, with its agent job skipped. The cap preserved a trail, but that run did not investigate or resolve the failures.
The Review-and-revision loop improved the gateway fix before merge. A review flagged missing WASM golden fixtures; the follow-up produced fixture changes, followed by a request to integrate main and recompile. The later successful PR reviews show the review machinery working again, without proving those reviews caused the fleet recovery by themselves.
Human Interventions That Mattered
Landon Cox filed #68881 with a specific startup failure and then asked Copilot to address review feedback on #68882. Peli de Halleux asked for main to be merged and conflicts resolved, then merged the repair at 00:46 UTC. Peli's earlier merged #68623 also has its first successful Issue Arborist follow-through in this window. Release dispatches were human-triggered; the logs support their order and outcomes, but not the reason for the two tag-step failures.

Episode Highlights
A shared gateway fault became a review-led repair. PR Code Quality Reviewer §38647400879 and Daily Go Test Parallelizer §38648224390 both failed at gateway startup. The failure also reached Release and dozens of smoke workflows. After the fix merged, PR Code Quality Reviewer §38679979380 completed and submitted a review, while Daily Go Test Parallelizer §38694711650 completed with a no-op. Outcome: improving; high confidence for the shared failure step and representative recovery.
