The daily breaking-change analysis identified five major, user-impacting changes in commit 8ba1171 that were not yet consolidated in CHANGELOG.md. This PR adds a single, scannable Unreleased → Breaking Changes section covering all five with migration guidance.

Changelog updates (Unreleased / Breaking Changes)
- Added entries for:
  - app: → github-app:
  - supportsLLMGateway → llmGatewayPort (explicit port required)
  - status comment no longer implicit (status-comment: true required when desired)
  - sandbox: false removal in favor of sandbox.agent: false
  - MCP Gateway v0.1.5 stricter server validation rules
Migration guidance included per breaking change
- Captured actionable upgrade instructions inline for each change.
- Explicitly called out codemod usage (gh aw fix) where available.
- Added compile-time validation recommendation (gh aw compile) for MCP config migrations.
Wording alignment
- Adjusted headings/text to reflect actual semantics (replacement/removal vs simple rename where applicable).
