v0.42.8-alpha: Add status bar item button for inserting headerbtn

This fork of https://github.com/kube/vscode-42header currently does not exist on the market place:
To install download the vsix file
Then in vscode use the Extensions: Install from VSIX... command in the command pallet.
I have no idea if this will work for you, if it doesn't please let me know!
Changes (compared to fork):
Fix newline deletion after 42 header
Add status bar button to quickly disable updating the header on save
Known Issues:
CRLF line endings currently do not work, use LF (it's better anyways)
