Adds a Codex deeplink/OAuth/external-launch detector and fixture for callback and activation regressions.

Highlights:

  • Detects codex://oauth_callback, Windows notification type=click&tag, AppX/MSIX protocol registration, mobile deeplink, browser-extension activation, and codex app <path> routing evidence.
  • Adds trace-to-skill demo deeplink-launch for zero-setup report reproduction.
  • Updates benchmark, scorecard, issue map, failure taxonomy, discovery docs, llms.txt, and OpenAI OSS brief to 29 public fixture cases.
