Document per-item professional payment hold (Professional Payments Report) Adds a new article for the per-item hold/release screen and cross-links it from the whole-BHT hold, admission dashboard, and payment-screen articles. Also fixes the whole-BHT Hold Professional Payments button's panel location in the dashboard article (it lives in Billing, not Clinical Actions).
