Fix hang on bad API key, resilient system doc migration

  • Voyage API auth errors (401/403) fail immediately instead of retrying 5x with exponential backoff (~31s of sleeping per embed call).
  • System doc migration is best-effort — if embedding provider fails at runtime, migration defers instead of blocking the user's write operation.
