Put Output in SDTM Format

Put the parameter outputs in SDTM format (http://www.cdisc.org/sdtm) so that they can most easily be included in a regulatory submission format.

All aspects of data format conversion will be moving to the PKNCA.portation package. This issue is migrating there.
