fix: reject stale CDP targets via JS liveness probe

  • Add probeCdpAlive() that sends Runtime.evaluate("1+1") after WS handshake; reject targets that don't reply within 1.5s
  • scan_metro now flags zombie pages with a STALE prefix and advisory, keeping scan_metro and get_apps in sync
  • Disambiguate logbox detection failures (dev_false, no_get_modules, modules_not_iterable, logbox_module_not_found, execute_failed, exception) so degraded-runtime bugs are diagnosable from the first error message
