Extend IHMCIF to add model_id to existing PDBx/mmCIF tables

With IHMCIF supporting heterogeneous model composition, model_id is required for some of the existing PDBx/mmCIF tables e.g., pdbx_poly_seq_scheme. Other tables will be identified after further discussion.
