fix: Simple mode shows pack/profile selection with defaults pre-selected

Simple mode now goes through pack and profile screens (so user can
change agent type and profile) instead of skipping directly to BuildPlan.
Defaults (openclaw, builder) are pre-selected. After profile selection
in Simple mode, method (terraform) and region (us-east-1) are auto-set
and it skips straight to review.
