fix: unique IAM role names + terraform install fallback for CloudShell

1. Default environment_name now includes an 8-char random suffix
   (e.g. loki-openclaw-a3f1b2c4) to prevent IAM role collisions
   when multiple installs target the same AWS account.

2. Terraform binary install falls back to /tmp/.local/bin when
   ~/.local/bin is on a read-only or space-constrained filesystem
   (fixes CloudShell 'not enough room' error).
