- Delete setup.ts (fully redundant with doctor) - opencli setup now prints deprecation warning and delegates to doctor - doctor auto-starts daemon if not running (no more false 'not connected') - Update all doc references (README, SKILL.md, docs/)

opencli setup，统一用opencli doctor。改动：
setup.ts（与 doctor 完全重复）opencli setup现在打印 deprecation 警告并转发给 doctordoctor自动启动 daemon（不再显示虚假的「未连接」状态）
