- 彻底删除 setup 命令（不保留兼容）
- doctor 默认开启 live 连通性测试
- 新增 --no-live 可跳过
- Remove setup command completely (no backward compat needed) - Doctor now runs live connectivity test by default - Add --no-live flag to skip if needed - Update SKILL.md docs
